Alright there has been many confusion on this before.

Official request layout: (Doesn't have to be used but is recommended for best results)

Code:
Type of Graphic: (Signature, Avatar, Banner, Userbar, etc.)
Image: (Not necessary but will help the artists)
Text:
Fonts: (Post links to the font you want if you want a certain font)
Notes:
